With time, the two sides of this muscle drift apart. When that happens, you get the vertical rope-like bands. You lose the clean angle under your chin. The whole area starts to look slack.

We go in, repair the muscle, remove the skin that has become redundant, treat the fat where it has accumulated, and restore the architecture that time quietly dismantled. Genuine Assessment Before Any Decision
When you come in for a consultation, we look at your neck properly. We assess the muscle, the skin quality, the fat distribution, the bone structure underneath it all. And then we tell you honestly what we see and what we think will actually help.

Your neck has layers. Skin on the outside. Fat beneath that. Then the platysma muscle. And below the muscle, deeper fat, smaller muscles, and other structures that can all contribute to how the neck looks and feels. A surgeon who only works on the skin will get a result that looks tight for a while and then relaxes back. We plan across all the relevant layers because that is where lasting results come from.
Depending on what your anatomy shows us, we may use one or a combination of the following:
Skin removal (cervicoplasty): When there is genuinely excess skin creating folds and sagging, we remove it through discreet incisions placed behind the ears and under the chin. The placement is deliberate so scars heal into the natural creases of your anatomy.
Muscle repair (platysmaplasty): The separated platysma edges are brought back together and secured with sutures. This is the structural core of the operation. It is what eliminates the bands and restores the support that gives the neck its shape.
Corset repair: A more focused technique for prominent central bands. A small incision under the chin is all it takes to tighten the midline and produce a visibly cleaner neck.
Fat treatment: Where fat under or within the deeper layers is adding bulk and obscuring the cervicomental angle, we address it selectively. Liposuction alone cannot fix a banded neck. But when used as part of a complete repair, it makes a meaningful difference to the final contour.
We decide on the combination during planning, not on the day. By the time you go into surgery, every detail has already been worked out.
The Day of Your Procedure
You will arrive knowing exactly what to expect because we will have walked through it with you already.
You will be given either general anaesthesia or sedation with local anaesthesia. The decision on which is made together, based on the extent of the procedure and what feels right for you medically. Our anaesthetic team is involved in your care from the beginning, not just on the day.
The surgery itself runs for roughly two to four hours. Incisions are placed with precision, the muscle work is done, skin is trimmed, and everything is closed with fine sutures. Most patients go home the same day. Some stay overnight if the procedure was more extensive, and that is planned for already rather than decided last minute.

Recovery: What It Really Feels Like Week by Week
We tell our patients what recovery genuinely looks like rather than painting an unrealistically easy picture.
The first week is about rest. Your neck will feel tight and there will be bruising and swelling. Tightness is the dominant sensation for most people, not sharp pain. Keep your head elevated when you sleep, skip the alcohol and aspirin, and do nothing strenuous. Any small drainage tubes are usually removed within the first couple of days.
Weeks two and three are when things start to feel more manageable. The bruising yellows and fades. The swelling settles. Most people feel comfortable being seen in public again and returning to desk work. The neck still feels firm but the anxiety of the early days passes.
Months two and three are genuinely exciting for most of our patients. This is when the result starts to become fully visible. The swelling has gone. The cervicomental angle is crisp. The jawline has definition again. Patients who were nervous in week one are usually the most enthusiastic by month three.
Some stiffness can linger past this point. It is normal and it resolves.
How Long Will the Result Last
Because we repair structure rather than just pull surface skin, the results hold up meaningfully over time. Most patients enjoy the benefits for many years, and some for considerably longer.
What happens afterward matters. Sun exposure breaks down collagen and accelerates skin aging. Smoking compromises healing and skin quality over time. Aging does continue. But a neck that has been properly repaired ages from a much better starting point.
