The 75 Gram Glucose Tolerance Test, Step by Step
- Fast overnight. Eight to ten hours with nothing but water. Book a morning slot so you are not fasting through the afternoon.
- First blood sample. Taken as soon as you arrive, before anything else. This is your fasting level.
- The glucose drink. A measured sugar solution, finished within five minutes. It is very sweet and most women find that the least pleasant part.
- Then you wait. Sitting in the clinic, not walking around, because activity changes your blood sugar and affects the result.
- Further samples. Usually at one hour and two hours after the drink.
- Home. Bring a snack for afterwards, since you will be hungry. Results typically come back within one to two days.
Bring something to read or a laptop. Two hours in a waiting room passes slowly otherwise, and you cannot leave and come back.
One-Step or Two-Step Glucose Tolerance Test?
Clinics use two different approaches, and it helps to know which one you are having. The one-step method is a single 75g glucose tolerance test after fasting, with samples at fasting, one hour, and two hours. One raised reading is enough for a diagnosis. This is the approach we use, and it means one appointment rather than two.
The two-step method starts with a non-fasting glucose challenge, which is a shorter screen. If that comes back raised, you return for a longer fasting test to confirm. It avoids the long test for many women, at the cost of a second visit for some.

Glucose Tolerance Test Normal Range in Pregnancy
Women search for these numbers constantly, usually while waiting for a result. Here they are, with the caveat that your doctor reads them in context rather than in isolation.
For a 75-gram test, the commonly used thresholds are:
- Fasting sample. A reading at or above 5.1 mmol/L, which is 92 mg/dL, is considered raised.
- 1 hour glucose tolerance test normal range. Below 10.0 mmol/L, or 180 mg/dL. At or above that counts as raised.
- 2 hour glucose tolerance test normal range. Below 8.5 mmol/L, or 153 mg/dL. At or above that counts as raised.
One raised value is enough for a gestational diabetes diagnosis on the one-step test. You do not need all three.
Two things worth knowing. Thresholds differ slightly between international guidelines, so a number that is diagnostic under one set may be borderline under another. And labs report in different units, which is why a figure you find online may look nothing like yours. Ask us which guideline and which units your result uses before you try to interpret it yourself.
How to Test for Gestational Diabetes at Home
The honest answer is that you cannot, and this is worth being clear about because the search suggests otherwise.
Home glucose monitors are designed for managing a confirmed diagnosis, not making one. A finger prick gives you a snapshot of your blood sugar at that moment. Diagnosis depends on how your body responds to a measured glucose load over two hours, in controlled conditions, with a fasting baseline. A home monitor cannot replicate that.
Once you are diagnosed, home testing becomes central to your care and we will set you up with a monitor and targets. Before diagnosis, a normal home reading tells you very little and a high one tells you to come in. Neither replaces the test.
