Fetal growth monitoring uses ultrasound to check your baby’s growth and development. The scan measures head circumference, abdominal circumference, and femur length. These measurements help estimate your baby’s weight. Doctors then compare the results with growth charts based on gestational age.

What Fetal Growth Monitoring Involves
Measuring your bump from 24 weeks gives a rough idea of growth, nothing more precise.
For real detail, we use ultrasound. The scan checks the head, abdomen and thigh bone and turns those numbers into an estimated weight and a centile. One scan gives us a snapshot. Several scans, two to four weeks apart, show us the trend, and the trend is what we watch.
How We Estimate Your Baby’s Weight
Four measurements go into the calculation.
- Head circumference and the width across the head
- Abdominal circumference, which is the most sensitive to growth changes
- Femur length, meaning the thigh bone
Estimated Fetal Weight (EFW) Calculation and Growth Charts
Those numbers feed into a formula that produces your estimated fetal weight, usually shortened to EFW. The Hadlock formula is the long-standing standard for that calculation, and it is what sits behind any fetal growth calculator you find online. We then plot the result against Fetal Medicine Foundation centile charts, which is the reference standard our sonographers work to.
EFW and Normal Range in Pregnancy
Skip past the weight for a second and find the centile. That’s the useful bit. Your baby could be on the 20th, the 50th or the 80th and still be perfectly normal. Only readings below the 10th or above the 90th lead to extra checks.
Even then, we care more about where your baby has come from. Steady on the 15th all along? Usually fine. Dropped to the 15th from somewhere much higher? We’ll want another scan soon.
Fetal Growth Chart by Week in kg
Average weights run from around 0.3 kg at 20 weeks to roughly 3.4 kg at 40 weeks, though the spread around each figure is wide. The centile table further down gives the fuller picture, and your own scan report is the only comparison that actually means anything.
Which Growth Chart We Use
Charts differ, and which one your clinic uses changes your centile. It is worth knowing which standard produced your number. Our sonographers work to Fetal Medicine Foundation centile charts. The FMF standards are widely used internationally and they are what every scan here gets plotted against, so your centile stays consistent from one appointment to the next.
You may come across others while searching. The WHO fetal growth charts describe growth in optimal conditions across several countries. The NICHD standards publish separate charts by ethnic group, because average growth genuinely varies between populations. Customised charts go further again and adjust for your height, weight, background and previous babies.
Comparing Different Fetal Growth Charts
That variation matters in Dubai more than most cities. A baby on the 8th centile against one chart can sit on the 20th against another built around a smaller mother. Comparing your centile to a figure from a different chart is how women frighten themselves unnecessarily, so ask us which standard your report uses.

Notice how wide each row is. At 36 weeks there is a full kilogram between the 10th and 90th centile, and every baby in that range is growing normally. That spread is the single most reassuring thing on this page.
Why One Fetal Growth Scan Is Never Enough
This is the most useful thing on this page, so it gets its own section. Ultrasound estimates of fetal weight carry a margin of error of roughly 10 to 15 percent. Therefore, on a baby estimated at 3 kg, that is a spread of several hundred grams in either direction. The figure on your report is genuinely an estimate, not a measurement.
So a single scan showing the 9th centile does not mean your baby is small. It means your baby might be, and the next scan will tell us considerably more. Two or three measurements plotted over time cut through most of that uncertainty, which is why we schedule serial scans rather than acting on one result.
Anyone giving you a firm conclusion from one growth scan is overstating what the technology can do.
Fetal Growth Calculators Online
Plenty of women arrive having typed their measurements into an online fetal weight calculator, and that is understandable.
Those calculators run the same formulas we do, so the arithmetic is usually right. What they cannot do is tell you which chart to compare against, whether your measurements were taken accurately, how your baby was positioned, or what your previous scans showed. They also cannot see your placenta or your Doppler readings.
Bring your numbers to the appointment by all means. We would rather go through them with you than have you interpreting a centile alone at midnight.
When Baby Growth Needs Closer Watching
Our doctor will recommend more frequent scans if:
- Exact bump measurement does not match your dates
- Your baby’s centile has dropped between scans
- You have high blood pressure, diabetes or an autoimmune condition
- You are carrying twins or more
- A previous baby was small or growth restricted
- You smoke, or you did before pregnancy
- Fluid levels around your baby look low or high

If Baby Growth Slows Down
Slow growth has causes worth finding rather than simply watching.
Most often, the placenta is not delivering nutrients as efficiently as it should. Doppler studies check blood flow through the umbilical cord and show us how hard your baby is working, which is more informative than weight alone. Where flow looks normal and growth has simply plateaued, we watch. Where flow is affected, the conversation turns to timing.
Sometimes the answer is a healthy small baby and nothing more. Constitutionally small babies exist; they follow their own centile faithfully, and they do perfectly well. Our job is to tell those apart from genuine growth restriction, and serial scans plus Doppler are how we do it.
If Your Baby Is Measuring Large
A baby above the 90th centile raises different questions. First we check for gestational diabetes, since extra sugar crossing the placenta is the most common reason for accelerated growth. If your earlier gestational diabetes screening came back normal, your doctor may repeat it.
Then comes delivery planning. A larger baby increases the chance of a difficult vaginal birth, so your obstetrician will discuss timing and method with you well before your due date. Remember the margin of error here too. Plenty of babies estimated at 4 kg arrive weighing 3.5, and decisions get made on the whole picture rather than one figure.
